ASH Choosing Wisely® Campaign: 2019 ASH Choosing Wisely Champions

In cooperation with the ABIM Foundation, ASH has introduced the Choosing Wisely Champions initiative to recognize the efforts of practitioners who are working to eliminate costly and potentially harmful overuse of tests and procedures. Each year, three individuals are invited to present on successful utilization strategies that they have developed and implemented in their practice, institution, or hospital system. This provides an opportunity for clinicians, administrators, and other medical professionals to learn about projects that might be translated to their own practices.

ASH Choosing Wisely® Campaign: The ASH-ASPHO Pediatric Choosing Wisely List

The Choosing Wisely initiative aims to help physicians and patients engage in conversations to reduce overuse of tests and procedures and supports physician efforts to help patients make smart and effective care choices. ASH has partnered with the American Society for Pediatric Hematology/Oncology (ASPHO) to develop a new, pediatric-focused Choosing Wisely list, scheduled for publication in conjunction with the 2019 ASH Annual Meeting. The list includes five tests, treatments, and/or procedures that are often overused or misused and that should be the subject of conversations with the patient. The session will include a presentation of each list item by an expert in the field.

ASH Foundation Run/Walk

Start your day in a fun, healthy, and philanthropic way by participating in the sixth annual ASH Foundation Run/Walk before attending the Sunday sessions on December 6. Participants may run or walk through the designated event course in scenic San Diego, choosing either a 3K or 5K route.

Invite your friends and family to support your efforts by donating to the ASH Foundation in honor of you or your team!

ASH and the ASH Foundation may opt to designate a portion of or all corporate sponsorship revenues associated with the run/walk to help support the cost of administering the event.

Schedule and Race Route

The ASH Foundation Run/Walk will start and finish at the Embarcadero Marina Park South and will take participants along San Diego’s scenic Embarcadero. Shuttle bus service will be provided from hotels on the meeting shuttle route. Further details will be available closer to the event date.

ASH Networking Reception for the Hemostasis and Thrombosis Community

This reception immediately follows the Special Symposium on the Basic Science of Hemostasis and Thrombosis and is a chance to network with colleagues from the hemostasis and thrombosis community. Clinicians and researchers are encouraged to attend.

Genomic/Genetic Testing: The Who, What, Where, When, & Why

Emerging evidence indicates that genomic data can be useful in all aspects of clinical practice, including diagnosis, prognosis and prediction of response to specific treatments, as well as in the development of novel targeted treatments for patients with hematological disorders. However, the rapid accumulation and complexity of evidence regarding the role of molecular testing in hematology has outpaced clinicians’ understanding and adoption of these advances, limiting their ability to apply these findings to their patients. There are several key issues preventing optimal use of genomic/genetic testing by healthcare professionals, including a lack of clarity on terminology and types of tests, a lack of confidence in interpreting and applying the results to actual clinical care, as well as their implications on treatment decisions. This session will first highlight basic concepts in genomic/genetic testing. The application of molecular methods in benign and malignant hematologic diseases will be discussed using a case-based approach, with the aim to illustrate when tests should be ordered, how to understand and interpret the results, and what to do about them.

The editors of the Blood journal highlight the intersection of hematology and the humanities in bringing to light the epic story of blood. This year Blood editors have again selected an author whose work showcases the medical professions impact on our cultural fabric. The Spirit Catches You and You Fall Down: a Hmong Child, Her American doctors, and the Collision of Two Cultures by Anne Fadiman explores the clash between a small county hospital in California and a refugee family from Laos over the care of Lia Lee, a Hmong child diagnosed with severe epilepsy. Lia’s parents and her doctors both wanted what was best for Lia, but the lack of understanding between them led to tragedy. Winner of the National Book Critics Circle Award for Nonfiction, the Los Angeles Times Book Prize for Current Interest, and the Salon Book Award, Anne Fadiman’s compassionate account of this cultural impasse is literary journalism at its finest.

Anne Fadiman is the Francis Writer-in-Residence at Yale. The Spirit Catches You and You Fall Down was her first book. Fadiman is also the author of two essay collections. The London Observer called “Ex Libris” “witty, enchanting, and supremely well-written.” NPR said of “At Large and At Small,” “Fadiman is utterly delightful, witty and curious, and she’s such a stellar writer that if she wrote about pencil shavings, you’d read it aloud to all your friends.”

The ASH Consultative Hematology Course (CHC) is an interactive, half-day program led by faculty familiar with consultative practice issues and commonly encountered clinical problems that require the expertise of a hematologist. Promoting Minorities in Hematology Presentations and Reception

The reception will also feature poster presentations by students participating in two Minority Research Supplement programs from the National Institutes of Health (NIH) as well as an announcement about the ASH-AMFDP Award, a partnership between ASH and the Harold Amos Medical Faculty Development Program of the Robert Wood Johnson Foundation. Special Education Session on ASH Clinical Practice Guidelines on Acute Myeloid Leukemia in Older Adults

In 2017, after a formal topic nomination and selection process, ASH initiated its first guideline development effort on a malignant topic: acute myeloid leukemia in older adults. This topic was selected as clinically important and largely unaddressed by existing guidelines. In 2018, a guideline panel composed of research experts, clinicians, and a patient representative prioritized clinical questions. Systematic evidence review is supported by the McMaster University GRADE Centre. By December 2019, draft recommendations are expected to be available for public comment.

In this special session, speakers will highlight particularly notable recommendations and will discuss important considerations for the practical application of the recommendations in practice.

A third edition of the ASH guidelines on immune thrombocytopenia are expected to be published in 2019. These guidelines, which address the management of both children and adults, update prior editions published in 1996 and 2011. The new edition was developed using updated methods, by an international panel that included research experts, clinicians, and patient representatives. Systematic evidence review was supported by the University of Oklahoma Health Sciences Center.

In 2016, ASH initiated the development of five clinical practice guidelines on sickle cell disease, addressing acute and chronic complications of the disease. Development of the guidelines, including systematic evidence review, was supported by the Mayo Evidence-Based Practice Center. Five guideline panels, which included more than 70 sickle cell disease experts and 10 patient representatives, reviewed evidence and formulated over 50 recommendations. The five guidelines are expected to be submitted for publication in 2019.

Six ASH clinical practice guidelines on VTE were published in 2018 in Blood Advances, and four others are planned to be published in late 2019 or early 2020 (prevention of VTE in surgical patients, treatment of VTE, prevention and treatment of cancer-associated thrombosis, and thrombophilia testing). Development of the guidelines, including systematic evidence review, was supported by the McMaster University GRADE Centre. The 10 guidelines will provide over 200 recommendations on VTE.

Special Symposium on Quality: ASH EQUIPS - Learning from Medical Errors and Other Incidents

Medical errors and preventable adverse events are widely recognized as important contributors to patient mortality, morbidity and experience. Incident reporting is an important tool for recognizing adverse events and near-misses, creating a culture of safety, and ultimately driving down the incidence of iatrogenic harm to patients. This session will explore incident reporting systems and how they contribute to creating safer care.

The first speaker will highlight the importance of creating a culture of reporting and incident learning through the review of a very public campaign in pediatrics. The second speaker will highlight the value of incident learning as illustrated by the Hemovigilance program, a transfusion incident reporting system.

The latter half of the Quality Symposium will introduce audience members to the ASH Empowering Quality Initiatives in Patient Safety (EQUIPS) Trainee Competition. Residents and fellows were invited to submit their response to the question “What is one change in clinical practice or care delivery that could improve the safety of care for patients with blood diseases?” Three finalists were selected from over 70 submissions and have been invited to deliver a five-minute presentation making the case for their suggestion. The panel co-chairs, speakers, and the audience will have the opportunity to ask questions of the presenters, after which the audience will select the winning presentation.

Special-Interest Session on Sickle Cell Disease Centers

Care delivered through specialized clinical centers has resulted in demonstrated improvements in patient care outcomes for hematological and non-hematological diseases alike. Unfortunately, the formation of specialized centers for the care of adults living with sickle cell disease has lagged behind advances leading to the potential for greater longevity and improved quality of life as well as the recognition that the transition out of pediatric care can be a significant event leading to early mortality.

In November 2019, ASH will convene a workshop to help interested institutions advance their plans to create specialized clinical centers for adults living with sickle cell disease. To extend these efforts beyond that workshop, this special interest session will address topics important in the formation of specialized centers, potentially including sessions on institutionalizing approaches to pain management, securing funding from the C-suite and other sources, and engaging patients, the community, and other stakeholders to achieve success.

Systems-Based Hematology: How to Implement Evidence-Based Programs for Virtual Hematology Consults and Anticoagulation Stewardship

This session will focus on highlighting the successful work of systems-based hematologists as they improve care delivery for patients with blood disorders, focusing on specific implementation of two high-impact projects. TRTH Scientific Symposium: A Decade of Training the Future Leaders in Translational Research

A TRTH Scientific Symposium will be held at the ASH Annual Meeting to celebrate the Translational Research Training in Hematology 10-year anniversary. The Symposium will give TRTH alumni the opportunity to come together, network, and hear about career successes since the TRTH program. The symposium will be open to any ASH attendee interested in translational research and will feature four TRTH-alumni scientific-research presentations.

Four participants whose projects exemplify outstanding translational research have been chosen to present. Each speaker will have 15 minutes to discuss a recent successful research project and scientific accomplishments of the project. The speakers will also comment on how the TRTH curriculum and mentorship component impacted their work and careers in translational research and are encouraged to share notable professional accomplishments and promotions they have received since participating in TRTH. After the presentations, 30 minutes will be allocated for questions, discussion, and networking.
